In this particular
case, the plate will be used as a reduction aid. That is, the final
fraction reduction is accomplished after
the plate is attached. Here, the plate is placed
posterior to the biceps. This particular plate
has a curve and a twist. Allows it to be
placed proximally, posterior to the biceps,
along the greater tuberosity. And then, more
distally, it is placed between the pectoralis
and deltoid insertions onto the shaft. Here is the provisional
reduction of the head fragment to the plate. At this point,
judgement of the height of the plate relative to the
proximal fragment is paramount. The plate is provisionally
secured to the shaft, again, prior to final
reduction of the main fracture. Next, the plate is further
secured to the head fragment. The vector of this clamp
allows compression of the plate to the head fragment
and also pulls the head into slight valgus,
relative to the plate. After provisionally securing the
plate with additional K Wires, the position of the reduction,
relative to the plate, is evaluated with fluoroscopy. Internal and external
rotation views confirm satisfactory
positioning of the plate.

foam mattress. When I heard about their topper, I was really
excited to try it out. See if it has that classic Tempur-Pedic feel. I want to talk about the construction, my
experience with it, who it’s a good for, and who it’s not a good fit for. Is it the right fit for you? Let’s find out. [music] Marten: I want to talk about the design of
the TEMPUR-Topper Supreme. First I want to say, if you want to see a
list of my top picks for mattress toppers, just go to Mattress Clarity, best mattress
topper. Let’s look at the TEMPUR-Topper Supreme. First off, it’s going to be three inches thick. On the outside from the top cover, you’re
going to have a polyester blend. On the bottom is going to be cotton, polyester
blend as well. More of a canvas feel. Going to give the topper some traction so
it won’t move around quite so much on top of your mattress. You probably want to know what’s inside. Let’s take a look. The cover is actually removable, which is
awesome. You can wash it at home. Keep this topper clean. A very, very nice option. On the inside, you have three inches of a
TEMPUR material. This is a very soft memory foam. Slow moving even for a memory foam. Very, very slow moving. Also, very high density as well. In terms of size options, you have twin, twin
long, double, queen, king and California king. Now I want to talk about my experience with
the TEMPUR-Topper. First off, you get this topper at your house
in a box. You take it out. It is going to be rolled up. You take off that thick plastic. Put it right on your bed. It is a softer memory foam. It takes a little bit of time to expand, but
I was able to use this topper that night. Also, in terms of off gassing not a lot of
off gassing. Not too much of that chemical smell. Very nice in that respect. To test out the firmness of this topper, I
put it on top of a mattress that was close to a 9 out of 10 in terms of firmness. This brought the mattress down to a 6 out
of 10. A very soft mattress topper. That is three points of firmness taken off
the mattress. In terms of sleeping position, starting on
my back was a very nice match for me. The mattress pillow was too firm for me when
I was on my back. This softens it up quite a bit. My hips sink in the right amount. Also, some nice contouring and this foam presses
up into my lumbar area. Very, very nice for back sleeping. Moving to my side. This topper really shines. Lying on my side, I am feeling almost no pressure
on my shoulders and my hips. I’m a larger person. A lot of times, I press right through toppers. I hit the mattress beneath, so I don’t feel
the best pressure relief. With the TEMPUR-Topper here, not the issue. Amazing pressure relief. Moving from my stomach, it is softening the
mattress quite a bit. I’m bowing at the hips. Getting out of alignment. Not the best match for stomach sleeping. In terms of feel, as I said before, it does
have this very, very soft slow-moving memory foam feel. Even for memory foam, a very slow-moving memory
foam. Very quick sandy. Also, very high density as well. Also a big issue with the memory foam toppers,
is warmth. They can trap heat. With the TEMPUR-Topper, I can say I was sleeping
a little bit warmer at night. Maybe not the best option for hot sleepers. I want to talk about the pros. Who I think this topper might be a good fit
for. First off, I think if you’re looking for something
for back sleeping this is definitely a good option. If you have your mattress that is way too
firm for you, your hips don’t sink in quite enough, going with the TEMPUR-Topper is definitely
a good idea. Softens up the mattress. Your hips will sink in a little bit more. You’ll feel some nice foam pressing up into
your lumbar area. It’s a very good choice for back sleeping. I think also this really shines again with
side sleepers. If you sleep on your side primarily, definitely
check out the TEMPUR-Topper. Softens the mattress quite a bit. I think even if you’re a larger person, you
won’t bottom out from the top. You’ll feel nice pressure relief on your shoulders
and your hips. Even if you’re a lighter person, a great option
there as well. If you like that memory foam feel, that classic
memory foam feel, Tempur-Pedic was that feel. That is that classic memory foam feel. Very slow moving. Very quick sandy. If you like to slowly sink into your mattress,
the TEMPUR-Topper is definitely a good option. Last but not least, if you sleep with a partner
you find yourselves waking each other up at night because of your movement. Take a look at the TEMPUR-Topper. With this thick memory foam, it’ll help cut
down on motion transfer so you and your partner will sleep better throughout the night. Let’s talk about the cons. Who I think this topper may not be the best
fit for. First off, if you sleep primarily on your
stomach you might want to look elsewhere. This is going to soften your mattress quite
a bit. This could cause your hips to bow in when
you’re on your stomach. You get out of alignment. Not the best match for stomach sleepers. Also, if you’re a hot sleeper this is probably
not the best choice for you. One of the things with memory foam like this
is it can trap heat. Actually direct it back at you. When you’re on top of this topper, you can
actually find yourself sleeping hot through the night. Also, if you have trouble moving around at
night, you have mobility issues, probably not the best topper for you. Again, it is that slow moving memory foam
feel. If you have trouble moving around at night
you’re worried about getting stuck in your mattress topper, probably not the best choice. Finally, right now at full price, this topper
is $400 for a queen size. I think it’s a good value when you consider
what you’re getting, but it might be outside of people’s price ranges. There are definitely more affordable memory
foam mattress toppers out there. In the end, I think this mattress topper is
a great option for back sleepers, side sleepers and those who want that slow-moving memory
foam feel. It also comes with a 25-year warranty and

Nothing is more common in childhood athletics
than sprains and strains and soft tissue injuries and in this case, we’ve got a soccer player
who obviously was coming up to try to get the ball, either rolled over the ball or kicked
another one of the players. But regardless, what we know about this mechanism of injury
is that there was a sudden, abrupt stop and the player collapsed to the ground. Now our
first assessment is obviously that he’s awake, he’s conscious, and he’s breathing normally.
He seems to be in a little bit of discomfort or even some mild to moderate pain. But he’s
not in an unstable situation. So immediate activation of EMS or 911 may not be our best
first option. We can do a little bit more of an investigation first. And right now,
what I want to know is, is this an injury that’s going to require this player to go
to the doctor or to the ER or is it actually a bump or a bruise that this player can deal
with and they just want to get back in the game. That’s the real question, isn’t it?
And so here’s how we’re gonna get to the conclusion of that, with a moderately safe and conservative
method of investigation. The first thing I’m gonna do is I’m gonna ask the player, do you
hurt anywhere else, other than, it looks like your knee? Do you hurt anywhere? And he shakes
his head no. This is all that hurts and he grabs his knee. We call this self-splinting.
They’ll do it to their arm. They’ll hold a rib. They’ll hold their knee or their ankle.
And in this case, he’s stabilizing his knee. What I want to do, in this case he’s wearing
shorts, obviously. But if they had pants on, I would take my scissors and I would cut the
pant leg to be able to reveal the injured part. Now what am I looking for? I’m looking
for bruising, swelling, deformity, bone ends possibly poking up through the skin or under
the skin. I’m looking for angulation that’s not normal. And then, I’m looking for, ultimately,
an open bleeding wound that has a bone end sticking out, which is the most serious of
these injuries when it comes to fractures due to infection risk and vasculature and
nerve endings and things like that. But what I’m gonna do is I’m gonna say, hey, could
you just let me take a look at your knee. You don’t have to move it. And you can see
that there’s grass stains and dirt. But I don’t see any of those symptoms that tell
me, this is a serious injury. I see no bruising, the skin color looks pink, and it looks normal.
I could actually take the shoe and the sock off, if it didn’t cause any more harm, and
I could actually look at the toes to make sure there’s good circulation coming to end
of the toes. There’s no blue toenail beds or purple toenail beds. They’re nice and pink.
And then I kinda wait to see, does this player want to get back in the game? Because they
kinda know what’s going on. They know if they’re really hurt or not. And if he starts wiggling
around and watching the other players or checking for mom and dad, that’s starting to indicate
to me that, yeah, it hurt. But it may not be a serious injury. If he stays here, keeps
splinting his own knee, that’s when I’m going to assist, holding the injury, and we’re gonna
activate emergency medical services and have the paramedics or EMS professionals come out
and take a second look. In most cases, if we have this situation going on, we may want
to stabilize it further before we move the injured player. So we don’t injure the knee
worse, we don’t cause any secondary problems with neurological deficit or nerve injuries
and, most of all, we want to make sure that this player is in as much comfort as possible
and we don’t aggravate the injury. Now let’s take a closer look at a real practical question.
Is it okay for them to return to work or play after an injury? And this is a very basic,
but a very common sense question that everybody needs to ask themselves, whether it’s for
school, for work, or for the home. You know, we know that he got hurt initially. But that
happens all the time. They rub it off. It kind of wears off. It wasn’t a serious injury
in the grades of injury. It was more of a shock. It kind of torqued it a little bit,
maybe it hurt initially. But now, as he’s sitting on the ground resting and I’ve asked
the questions, he begins to say, you know, this doesn’t hurt so bad after all. I can
tell that because the patient is starting to move their own limb. Remember initially,
if it’s really serious, they don’t want anybody to touch it. Don’t touch it. Don’t move it.
I’ve got it. So we call this self-splinting. That’s the way I found this player initially.
But now as we’re talking, he’s kinda paying attention to the other players in the background,
checking out the game, maybe looking for mom and dad, starts to joke and laugh with his
friends. And the next thing you know, I see him start to move his leg back and forth,
wiggling it around. And I say to him, hey, is it feeling better to you? He acknowledges
it is. And I say, would you like to try to just stand up for me and see if it hurts when
you try to stand? Just be real careful. You can use my hand if you need to. Take it easy
there. How does that feel, standing up, just standing? If it hurts seriously, then we’re
gonna have him sit back down. And I’m gonna help protect that leg, or we’re gonna get
a chair or a stool, or we might even help assist them off the field. But in this case,
I talked to him and he’s kind of walking around and looking like, yeah, it’s not so bad. Then
I say, well, why don’t you just do a little circle here, just kinda put some weight on
it. How does that feel? He says, feels pretty good. I feel, I don’t feel like it hurts as
bad anymore. And at this point, now time is on our side. And I say, looks like you’re
doing well. If it starts hurting again, then you come off field. And we’ll help you, okay.
Otherwise, go have fun. He takes off, gets back into play, or goes back to work or back
to school. And in the meantime, what we do is we educate parents, we educate the coach,
maybe we are the coach, and we make a note of that. There was an injury. We’re gonna
watch this player, see if it happens again, and if it does, that’s when we’re gonna to
really assess whether this player should be seen at the urgent care center or the hospital.

A break or crack in a bone is called a
fracture. They’re usually under the skin, these are closed fractures but if they break through the skin they’re called open fractures. Look out for swelling, difficulty moving, movement in an unnatural direction, a limb that looks shorter, twisted or bent, a grating noise or feeling. Loss of strength, shock. If you suspect a broken bone this is what to do: Tell the casualty to keep still. Help them to support their injured body part to stop it from moving. This should ease any pain and prevent any further damage. If it’s an open fracture, cover the wound with a sterile dressing and keep it in place with a bandage. Apply pressure around the wound to control any bleeding. Once you’ve done this call for medical help. While waiting for help to arrive, don’t move the casualty unless they’re in immediate danger. If you can do so without causing them further harm, protect the injured area using bandages. For example if it’s a broken arm, put it in a sling. This will help to hold it in place. Keep checking the casualty for signs of shock. If they become unresponsive at any point, you’ll need to go back to the primary survey starting at A for airway.

-We’ve helped a lot of people. But there’s still so much work
that needs to be done. I’m very excited for this trip. Providing access to clean
drinking water is a priority. But I also want to
broaden my impact. My plan is to link up with
Puerto Rican activists… …who are working to rebuild the
island and see how we can help. It’s been months since
the first time you’ve been here. Shit hit the fan.
There’s still shit everywhere. The water’s an issue
not only in drinkable water. There’s other issues.
There’s a lot of water stuck… …and it’s creating disease
and it’s creating mold. These filters effectively treat
contaminated water. But it’s not as simple
as handing them out. People need to be trained
on how to safely use and clean them. And that’s what I’ll be doing today. This will be my first time giving out
water filtration systems… …without Waves For Water.
I’m definitely nervous… …but I do have my boy,
Puerto Rico Rob, along for the ride. This filter can producer 100 gallons
of water per day. This is his first time. Oh, my. [Laughter] One of the women we just met
is taking us to see elderly man… …and his son whose house
was badly damaged by the hurricane. They have a tarp on their roof. Or the tarp is the roof now. -This is what Maria done.
-It broke down the whole shed? All– All of this. What does it cost to fix this? We can put this again
because we no having nothing new. We just…. I don’t know how much we can spend. And your name? Juan Sr.? -Mon. Mon.
-Mon? Like many in Puerto Rico,
Mon and his son… …have been relying
on bottled water. This filter will allow them to
safely drink the rain water. This is a grass roots
word-of-mouth effort. We meet someone, and they
introduce us to someone else. Our goal is to get this water
to the whole community. Doesn’t matter who is it.
Religion or political…. I’m just working for the people. -Thank you. I appreciate it.
-Yeah. Bye-bye. We are heading to Rincón
on the west side of the island… …to meet up with my friend, Angela. She does a lot to help the deaf
and hard-of-hearing community. The deaf are still without
basic needs. And that’s a problem. I made a Facebook video,
said I’m gonna stop here. One person saw the video, and they go
to another person’s house. “Hey, hey. Come on.” They come here
and meet to get supplies. Because they don’t have access.
There’s no electricity, no Internet. And they can’t just be informed
by phone. They really have to be, like,
person to person. You want this to be
a system that stays stationary. You don’t want it moving around. -Is it ready?
-It’s ready. All of them are ready. I grew up hearing,
and I lost my hearing… …when I was around a teenager. I typically bring out supplies,
sustainable supplies… …to different remote areas. And all my focus went
on Puerto Rico because… …my family lives here. And I grew up, not here,
but coming here. I didn’t really know the deaf
community in Puerto Rico until this. Now I–
Now I have a lot of friends here. We’ve been giving out Luci lights
everywhere we go. Since they’re solar-powered,
they’re really helpful… …to people who still don’t have
dependable electricity. Those of us who can hear well
often take communication for granted. But for the deaf,
losing power makes this… …basic necessity
extremely difficult. If you don’t have light to see,
you can’t use sign language. You go to somebody’s house,
and you wanna knock on their door. Something hearing people
take for granted. You knock on their door,
there’s no light, no electricity. They’re not gonna hear the knock.
We can go up. We might need light to communicate. You have the lights for her? There’s so much
that needs to be done. Trying to make it easier. If light is gonna enable you
to communicate at night Let’s do it.
-That works. Let me get a hug right here. Hurricane Maria not only
destroyed homes… …and much of the electrical
and telecom systems… …it also wreaked havoc
on farmers… …who lost nearly
all of their crops. Today, Rob and I are meeting up
with two food activists… …Tara Rodríguez-Besosa… …and Camille Collazo to find out
about their efforts to aid farmers. So we go to a farm,
and we just start to get to work. Whether it be a latrine
or a germination station… …or a roof or, you know,
set up a solar system. We have everything we need,
we camp out… …and we just help out in the farm. After Maria, we started a fund,
Regrow Puerto Rico… …that will give support to 100-plus
farmers from five different markets. So they could have some… …support monthly as work insurance
until they get back on their feet. Tara and Camille are concerned
about the big-picture-food situation. Currently, the island imports
more than 85 percent of its food. Within 10 years,
if we wanna produce… …at least half
of the food that we eat… …we just need a whole bunch
of different types of food systems… …that are working together. A lot of the work I’m involved in
is dealing with community gardens. Getting people to just start
growing their own food. You don’t need to be a farmer
or live outside the city to do this. We just need more people
growing food. And then that’s it. Every little support that we have
received is a pat on the back. Keep on going. And we’re really grateful. -Yeah. All right.
-See you. Today, we’re heading up to Caguas… …to work with Waves For Water
and local activist, Fernando Silva. This farming community
doesn’t have access… …to clean water,
so we’re installing… …two water cisterns,
each equipped with… …multiple filters to help them out. We found a spring up there.
The community found a spring… …and we’re basically gonna
siphon the water off the spring… …and gravity-feed it
into the cistern. And make a water depot
for the communities. The only way they can take advantage
of that amazing resource… …of having a spring
in the community… …is having a way to
filter that water. There’s a big secondary problem
that’s gonna come out of this. It’s all the plastic bottles
that have been shipped in. So there’s gonna be
a big trash problem. Feeling good about implementing
a system that– Yeah, we use plastic.
We use a bucket. But that bucket’s gonna have
a big life. Opposed to a bottle that’s gonna get
drank and thrown in a river. Thrown in the trash. Those are
the things we wanna try to avoid. The basis of any recovery
of any country… …is on hands of its own people. We have to provide the resources
to the community… …and then the community will be
the driver to solve the problems… …not only from the hurricane
but any other problem… …that we have to face
in the future. We could pay for a bunch of filters,
but without people like you… …it doesn’t get the full reach
that it could. -That’s right.
-That’s a great thing. This is what has blown me away
on this trip: Seeing all of these people
doing their part every day… -…to help Puerto Rico.
-We’re brothers and sisters. -We take care of each other.
-That is this idea of community. So when I venture around
the island of Puerto Rico… …I have support everywhere I go. If you empower the people to solve
the issues… …and look for initiatives that
they can better their community… …then I think we’re creating a
solution to that bigger problem. Thank you. I’m glad
we’re all part of the same team. -Yeah.
-I’m a proud Puerto Rican… …because I’ve seen the deeds
of these kind of Puerto Ricans. They are super heroes. They are the epitome of amazing. They were there
to lift their people up… …when they were in need,
and when they were in crisis. It’s gonna take years,
and it won’t be easy… …but together, we can rebuild
a stronger Puerto Rico.
